    1. Bio-Data form, duly attaching latest Passport size photo and the form shall be
    signed with name in capital letters.
    2. Attested copy of marks memo of SSC or equivalent certificate for proof of Date
    of Birth.
    3. Attested copies of MBBS Degree certificate and provisional issued by
    recognized University under MCI Act.
    4. Attested copy of Internship completed certificate.
    5 .Certificate of permanent Registration from A.P. Medical Council. Candidates
    acquired degree from other states / other countries
    must produce APMC Registration certificates.
    6 .Attested copy of marks memo of all years of MBBS.
    7 .Attested copy of latest caste certificate (in case of SC/ST/BC)
    8. Attested copies of study certificates from Class–IV to X where the candidate
    studied.
    9 .Attested copy of latest Physically handicapped certificate (if applicable).
